SPORTS PHYSICALS will be offered on Wednesday, July 26, 2023 from 5:30pm - 8:00pm for 7th - 12th Grade Athletes in the CMHS Gym. Cost will be $20.00 and the monies collected benefit CMHS Athletics. No appointment is necessary - first come, first served. All forms may be downloaded at www.cmsd12.org under the Athletics tab - Athletic Forms. If parent(s) are not present at the physical, please complete the medical release and disclaimer form. Please Note: Any Student wishing to participate in a sport next year must have a current physical BEFORE practice begins and the Physical may NOT expire prior to the official end of that season.
